Jessica's Story
Jessica needed support managing her care needs. She contacted her local council requesting an assessment. Unfortunately, she has been waiting for her council to contact her for nine months. She then heard about www.Findyoursupport.com through her friends and signed up for the FYS platform to be assessed under Sec 9 of the Care Act 2014. Soon after, her signup verification was successful. The process began with Christine, who conducted a Care Act assessment virtually or in person. If eligible under the Act, after successful verification and authorization, we worked in partnership with Jessica to create an outcome-focused support plan to manage her identified eligible needs under the Care Act more effectively. In the meantime, if her circumstances change or she has any questions related to the care provision, then all she does is contact Christine on her dashboard or her direct line for further support. Doing this has reduced the need for unnecessary calls to her local council for support under the Care Act.
